Who we serve

Program Overview

The Community Outreach Program provides free or low-cost services to residents living in low-income senior housing communities (55+) through education, volunteerism, case management, and strategic partnerships.

Our goal is to reduce isolation, improve health outcomes, prevent unnecessary hospitalizations, and connect residents with essential community resources.

A signature initiative

“Community Care Days”

One idea that could distinguish World Wide Community Outreach is hosting quarterly Community Care Days at 55+ apartment communities. Instead of offering one service, residents can access multiple supports in a single visit. Stations could include medication reviews, advance care planning, financial coaching, resource navigation, food distribution, mobility equipment cleaning, emotional support, and health education. This “one-stop” approach reduces barriers to access, strengthens community partnerships, and creates a welcoming event where residents receive comprehensive support in one place.
